‘MEMBER CHANGE REQUEST’ FORM REPLACES
ANNUAL ‘MEMBER INFORMATION UPDATE & REQUEST FOR SERVICES’ PROCESS
Each year, AYC Members received a ‘Member Information Update & Request for Services’ form which they were asked to complete and return to the AYC Office. The purpose was to ensure that AYC records remained up-to-date. However, over the years this process has proved less than satisfactory. The return rate of the form is poor, and for many Members who do complete and return the form it is often an unnecessary/repetitive exercise as there are no changes. Additionally, it takes time for the Office to compare each returned form to what is currently on file.
So, effective immediately (November 2016)…
1. The annual distribution of the existing ‘Member Information Update & Request for Services’ form is discontinued.
For records and billing purposes, a Member’s Profile, Boat Information and/or Boating Services Required will be assumed to remain the same from year to year unless the Member informs the AYC Office otherwise.
2. A ‘Member Change Request’ form has been created.
Whenever any of a Member’s Profile, Boat Information and/or Boating Services Required changes, the MEMBER WILL BE RESPONSIBLE to complete a ‘Member Change Request’ form and submit it to the AYC Office.
